** If you are a new patient, and or you have new insurance, please call the front desk to verify your insurance. It may take us longer than 24 hours to verify the insurance but we do try to get to it as quickly as possible. As a courtesy we do verify all insurance for patients, and bill patients insurance plans. Ultimately patients are responsible to know and understand your individual insurance plans. Your insurance plan is a contract between you and your insurance company; It is your responsibility to know your coverage. You are required to provide correct and updated information regarding your insurance. We cannot guarantee your insurance benefits or the information your insurance company provides.You are responsible for the payment of co-pays, coinsurance, deductibles and all other procedures or treatment not covered by your insurance plan. You are ultimately responsible for the payment of provided care and treatment.

KAISER / CHP MASSAGE PATIENTS: When booking a massage with Kaiser / CHP insurance please choose the Kasier / CHP massage type. You may have to scroll down the list a little bit to find it. When you select this type it will show you all of the providers who take this insurance type. We have a limited number of providers who take this insurance, and we want to be sure that you are able to get a massage with someone who does take your insurance so that there are no surprises on day of service.

MVA, MODA, BCBS, UHC, AETNA, PROVIDENCE MASSAGE PATIENTS: You can book under insurance massage. Please be sure that you are under the care of a Chiropractor at our clinic to make sure that your PT benefits can be billed correctly.
