LOCATION: We have 3 locations NE Alberta, NE Boradway, and SW Portland (S. Corbett Ave). Please choose the location you would like to book at, then choose the provider or treatment type you are trying to book. You can book by provider, or by treatment type. If you choose to book by provider, you will only see THAT providers offerings at the LOCATION you selected. If you choose to book by appointment type you will see openings with ANY providers for that appointment type at the LOCATION you selected. Please be aware of the location you booked at, our providers work across multiple locations.
INSURANCE You can check out the Insurance tab on our website. Please allow 24 - 48 business hours for front desk to verify your benefits. You are required to provide correct and updated information regarding your insurance. IF YOU PLAN TO USE INSURANCE BENEFITS PLEASE CHECK WHICH PRACTITIONERS ARE IN NETWORK WITH YOUR PLAN BEFORE BOOKING. Insurance credentialing is always updating as we welcome new practitioners to the team. IF YOUR INSURANCE HAS CHANGED SINCE YOUR LAST VISIT: You must send us your updated information at least 2 business days (Monday-Friday) prior to your next scheduled appointment. Please email our front desk!
NEW PATIENT APPOINTMENTS: Upon booking your first appointment at our clinic, you will receive an email with our intake paperwork where you can include your specific insurance plan information. IF YOUR LAST VISIT AT THE CLINIC WAS 3 YEARS AGO OR PRIOR: You must book a New Patient appointment.
